1) You must register within 30 days of purchase to be eligible for this program.

2) Once registered, the Ryze Relief will reimburse you for the REPAIR of your broken device screen based on the reimbursement amount of your original purchase (up to $200).

3) All receipts and images are run through AI software for authenticity. Claims using AI generated receipts and images will be denied.

4) Ryze does not replace phones, this program covers your screen repair only.

Before filling out the registration, please have the following available:

1) Your 15-digit phone IMEI number, this can be easily located by dialing *#06# on your phone's dialpad.

2) Sku/Item #, which is located above the UPC code on the back of the package
